It is highly recommended that you schedule 3-4 visits in the first 2 weeks of initiating treatment. Our initial visit will involve a detailed assessment and functional diagnosis, establish what your needs for treatment will be, and we will work on an appropriate plan. This is a lengthy process and early follow up ensures that we will be able to progress along the path we establish. Be advised that if you prefer to wait to schedule a follow up until your first visit there may be a significant break in your therapy until we are able to book your next session.
If you prefer to schedule by phone you are welcome to do so; However, as I will be working with other clients during the day, I may not be able to return your call until a later time. Please leave a detailed message and your call will be returned.

I am a credentialed provider for the following insurance companies:
The Premera Blue Cross Network which includes the Global, Heritage and Foundation Networks along with the Heritage Prime and Heritage Signature networks as a Tier 4 provider (highest benefit level). In Physical Therapy is also a primary provider with Lifewise, Regence, Uniform Medical, Aetna, First Choice, and Cigna. We accept State Labor and Industries claims and motor vehicle accident claims with open and authorized claims.
In Physical Therapy is not in network with United Health Care insurance. Patients without insurance coverage are eligible for a 25% discount if payment is made at the time of service.
We are not able to provide delayed payment plans in third party payor situations.
In Physical Therapy is not a Medicare provider.
Billing services are provided via an external contract and can not be addressed in the Seattle physical therapy clinic. Please contact Chapin's Medical Billing at (206) 243-2911 with questions or concerns regarding your bill.
Please Note: A $130 late cancellation or no show fee will be charged to any individual who fails to show up on time to their appointment or cancels with less than 24 hours advance notice. This fee is not payable by insurance.
I ask that you please let me know as soon as you are able if you need to cancel or reschedule a visit so that I can offer that time to another individual.
