Treatment hours are available Monday through Friday by appointment.

Scheduling online is the most direct way of finding a time that works for you.  We provide online scheduling through fullslate.com by clicking the above link.  Please let us know if you have any difficulty making or modifying an appointment.

If you prefer to schedule by phone you are welcome to do so; However, as we will be working with other clients during the day, we may not be able to return your call until a later time.  Please leave a detailed message and your call will be returned.

If you do not find times that work for your schedule, please call or email for assistance.

INSURANCE COVERAGE

We are credentialed providers for the following insurance companies:

The Premera Blue Cross Network which includes the Global, Heritage and Foundation Networks, Lifewise, Regence, Uniform Medical,  First Choice, and Cigna.  We accept state labor and industries claims with open and authorized claims.  You may also have out of network benefits for treatment if you have coverage through a plan not listed here.

Unfortunately Aetna has discontinued allowing 'new' therapist contracts in their system for the time being.  While this is disappointing, we will continue to seek a contract with Aetna to meet the demands of our clients.  In the interim we will be considered 'out of network' providers for this plan.

We are unable to bill secondary insurance, but you may submit claims independently based on your secondary insurance requirements.  We are also not able to provide delayed payment plans in third party payor situations.

In Physical Therapy is not a Medicare provider.  

Billing services are provided via an external contract and can not be addressed in the clinic.  Please contact Chapin's Medical Billing at (206) 243-2911 with questions or concerns regarding your bill.

Please Note:  A $50 late cancellation or no show fee will be charged to any individual  who fails to show up on time to their appointment or cancels with less than 24 hours advance notice. This fee is not payable by insurance.
